We also work with the food people actually eat. Matoke, posho, groundnut sauce and katogo can nearly always be adapted to a safe texture rather than abandoned, and people stick to a diet built around food they want to eat. Families do most of the feeding, so we train whoever is doing it: how to position someone safely, what to watch for, and what to do if they choke.

There are very few clinicians in this country who specialise in swallowing. Most patients with dysphagia end up managed by well-meaning staff who have not been trained in it, and the default advice is usually to thicken everything and hope. We reassess as recovery progresses, including moving people back onto normal textures as soon as it is safe. Stroke is a leading cause of disability worldwide according to the World Health Organization, and swallowing difficulty is one of its most common and most treatable consequences. Where speech is affected too, our speech therapy team works on both together.
